seven more years of rigorous training in general surgery. He then
participated in numerous fellowships, including two years of burn surgery at
New York Hospital, craniofacial surgery at Children’s Hospital in Salt Lake
City, Utah and aesthetic and reconstructive breast surgery at the Northern
Westchester Aesthetic Institute in Mount Kisco, New</span><span style="font-family: "helvetica"; font-size: 18pt;">York. Continuing with his
extensive education, he went on to complete a two year residency in Plastic and
Reconstructive Surgery at the prestigious Medical College of Georgia. He
has co-authored publications, including scientific articles and book chapters
about reconstructive surgery. Upon completion of his plastic surgery
training, Dr Magana was awarded the esteemed Kenna Given Award for his

<span class="Apple-style-span" style="font-size: large;">S</span>everal thousands of women undergo breast augmentation every year. Many of them in childbearing age. A frequent concern regarding silicone implants is whether or not the ability to breast feed is preserved, and if so, is it of any consequence to the baby. Fortunately, to date, there seems to be no major implication to the child in these circumstances. Studies analyzing the amount of silicone contained in the milk of lactating mothers when compared to commercial formulas have found little difference in the silicone content. The importance of breast feeding is undeniable. Medical literature has documented the nutritional and immune-stimulating properties of of maternal milk when compared to commercial formulas. Maternal milk seems to be particularly protective against common childhood conditions such as Eczema, Iron deficiency anemia and Otitis media. Other less common and more serious diseases seem to be unequivocally mitigated by breast feeding; upper and lower respiratory infections and lymphoma amongst others.<br />
For these reasons, it is recommendable to breast feed if the mother is able to do so. Approximately 50% of women are able to breast feed at baseline, and it is important for the physician to discuss this prior to undergoing breast augmentation with silicone or saline implants since it is unknown whether a patient is able to breast feed at baseline prior to the procedure.<br />

A study published in PRS in 1998 undertook the arduous task of comparing the levels of silicone in breast milk from lactating women with and without silicone implants.<br />
Two other sources were also included in the study consisting of cow's milk and infant formula for silicon content.<br />
The samples were prepared in a special class one hundred "ultraclean" laboratory and analyzed via graphite furnace atomic absorption spectrophotometry. Silicon levels were analyzed in breast milk, whole blood, cow's milk, and 26 brands of infant formulas.</div><div>-Lactating women with implants were found to be comparable to control groups when measuring silicon levels:</div><div>(55.45 +/- 35 and 51.05 +/- 31 ng/ml, respectively) </div><div>-In blood (79.29 +/- 87 and 103.76 +/- 112 ng/ml, respectively). </div><div>-The mean silicon level measured in store-bought cow's milk was 708.94 ng/ml, and that for 26 brands of commercially available infant formula was 4402.5 ng/ml (ng/ml = parts per billion)</div><div>-lactating women with silicone implants are similar to control women with respect to levels of silicon in their breast milk and blood. </div><div>-Silicon levels are 10 times higher in cow's milk and even higher in infant formulas.</div><div><br />
</div><div>In conclusion: there is no current evidence that would suggest that silicone implants have any undesirable effect on the child in the breast feeding period.</div><div><br />
</div><div><span class="Apple-style-span" style="color: orange;">-<span class="Apple-style-span" style="font-size: large;"> Do silicone implants reduce the ability to breast feed?</span></span></div>The breast tissue that produces milk is in reality a modified sweat gland. It lies over the pectoralis major muscle and is composed of units known as lobes. Each lobe is composed of lobules. Small ducts emanate from the lobules where the milk is produced and then channeled toward the nipple where they coalesce into several lactiferus sinus which then empty into the nipple. Silicone implants are always placed behind the gland and does not divide the ducts or damage them. If the lobules and ducts are intact, the production of milk shouldn't change. Of the four procedures to insert implants, only one may divide the breast tissue containing the ducts, namely, the Trans-areolar approach. In general, even this approach usually does not affect breast feeding.<br />

</span><br />
<span class="Apple-style-span" style="font-size: large;"><span class="Apple-style-span" style="color: cyan;">-Trans-Areolar:</span> </span>divides breast tissue, but usually does not affect lactation<br />
<span class="Apple-style-span" style="font-size: large;"><span class="Apple-style-span" style="color: cyan;">-Inframammary incision:</span> </span>This incision is made under the breast crease and introduces the implant behind the breast tissue, avoiding the gland alltogether<br />
<span class="Apple-style-span" style="font-size: large;"><span class="Apple-style-span" style="color: cyan;">-Trans-axillary Incision:</span> </span>In this method, the incision is in the axilla and the implant is once again introduced behind the breast tissue without damaging the glandular breast tissue<br />
<span class="Apple-style-span" style="font-size: large;"><span class="Apple-style-span" style="color: cyan;">-TUBA Trans-umbilical Breast Augmentation:</span> </span>this procedure is only appropriate for saline implants and it uses a scope for dissection from the umbilicus to the retro-mammary space (behind the mammary gland) and does not affect the gland.<br />
<br />
It has been documented that a women undergoing breast implant placement have a 2.6 to 3 times greater risk of not being able to breast feed than the rest of the population. This is in part attributable to the surgical approach which may be trans-areolar, and to the possible unwillingness of the mother to lactate after augmentation.<br />
It should also be mentioned that if a woman is already lactating and wishes to have breast implants, it is advisable to wait for 6 months prior to undergoing augmentation. Milk accumulation has been documented in the peri-prosthetic space after surgery when augmentation is performed earlier.<br />

It is uncomfortable to patients who suffer it and can result in depression in some cases. It may exacerbated by different emotional states such as nervousness or anxiety. In the stricter sense, Hyperhidrosis is sweating in excess of the required amount normally needed for thermoregulation.<br />
The ethiology (cause) of AH can be divided into primary and secondary. The primary has no recognizable cause for the excess activity of the Eccrine glands (Sweat Glands). Secondary Hyperhidrosis is caused by a concomitant illness or recognizable associated causes including certain medications.<br />

Prior to treatment, a thorough history and physical exam should be done to exclude any causes of secondary hyperhidrosis.<br />
Diagnistic tests for this condition include:<br />
<br />
-The Starch-iodine test. In this test, an iodine solution is applied to axillary area bilaterally. Once it has dried, starch is sprinkled on the site. The combination of excessive sweat and starch results in a dark blue color yielding a positive result in the presence of hyperhidrosis. This is often patchy in nature, revealing the most concentrated sites of sweat secretion<br />
<br />
<div>-In the Paper test. Special paper is placed on the affected area to absorb the sweat, it is then weighed to determine the amount of sweat contained in it.<br />
<div><br />
</div><div>-The most accurate to determine the presence of hyperhidrosis is with a device called a VapoMeter which provides an objective measurement for the amount of sweat. It measures the relative humidity in grams per meter square per hour and is quite accurate. Three measurements are done in the clinic and a control site such as the forearm is used as a control.<br />
<br />
<span class="Apple-style-span" style="color: cyan; font-size: large;">What are the treatment options?</span><br />
<span class="Apple-style-span" style="color: yellow; font-size: large;">Non Surgical treatments include:</span><br />
-Antiperspirants: Usually in the form of aluminum chloride hexahydrate which will occlude the outlet of the gland decreasing the actual perspiration versus a deodorant which does not address the amount of sweat being perspired<br />
-Iontophoresis: This method is FDA approved and uses a small electrical stimulus to reduce the amount of secretion in the affected area. It is most effective in the soles of the feet and the palms. The mechanism of action seems to be mild thickening of the regionally affected skin.<br />
-Systemic Medications:Usually in the form of Anticholinergics such as Glycopyrrolate which decrease the amount of sweat being produced. These medications may have side effects such as a dry mouth, Dizzines and sometimes produce difficulties urinating.<br />
-Botox: Type A botullinum type toxin is FDA approved for the treatment of Hyperhidrosis. It is generally effective in decreasing the symptoms associated to AH. It's mechanism of action consists in blocking the signal from the regulating nerve to the actual gland. Downsides to this treatment are that it is temporary and should be repeated every 3-6 months, may be locally painful and also produce a some local transitory weakness. It may be applied with a pre-fabricated grid shape, or with a more targeted fashion by following the the "Minor's Starch Iodine test enhanced sites which are sometimes scattered.<br />
<span class="Apple-style-span" style="color: yellow; font-size: large;">Surgical Options:</span><br />
-Endoscopic Thoracic Sympathectomy : This procedure is done via 2 incisions in the axilla. The sympathetic chain is localized next to the vertebral column anteriorly and then divided, thus cutting the nerve supply to the sweat glands. In the right hands, this procedure has a high success rate of 90-95%. There may be recurrence a small percentage of the time, or compensatory sweating in other parts of the body as a result of the sympathectomy.<br />
-Suction assisted Arthroscopic Shaving*<br />

*Definitive Diagnosis and Management of Axillary Hyperhidrosis: The VapoMeter and Suction-Assisted Arthroscopic Shaving.<br />
<div>This last procedure can be done in under an hour under general anesthesia and provides lasting effects by permanently excising the local sweat glands. It is done by marking the hair bearing area of the Axilla, after which 0.5% lidocaine is infiltrated for hemostasis (to reduce bleeding). A small incision is made in the crease area and the Arthroscopic Shaver is introduced and the eccrine glands (Sweat glands) are ablated by "Shaving" them off. This procedure does not damage the hair follicles.<br />

Asian eye lid surgery (blepharoplasty) has become an increasingly popular cosmetic surgery all over the world. A common erroneous belief is that the goal of this type of procedure is to achieve a "westernized" appearance, which is rarely the case. Instead, the goal is most commonly a natural and younger appearance.<br />

Often patients will seek this surgery because they feel that a heavy upper lid will limit their expressive ability and thus limit their ability to interact socially. This concept is often misconstrued by American, European and non-Asian individuals as a desire to appear more Caucasian.<br />

What are some of the characteristics of an Asian eyelid when compared to an occidental eye?<br />
-The presence in varying degrees of an epicanthal fold. When prominent epicanthal folds are present medially (on the nasal side) they can give the undesirable illusion of esotropia (cross-eyed).<br />
-An eyelid crease which is lower set and often not visible. When present, it differs from the non-Asian population in that is parallel to the lid margin and narrows as it reaches the nasal edge of the lid<br />
-The angle of the brow tends to be higher medially (Close to the nose) in Asian eyelids<br />
-Shallow orbits (eye bones)<br />
-Minimally or non Cantilevered supraorbital ridges<br />
<br />
The so called "double lid surgery" is the commonly requested surgery make the eyelid crease a more noticeable trait. This procedure has been analyzed over the years and modified many times over the years.<br />

Principles of the Asian "double lid surgery"<br />
This varies greatly between individuals since the presence of an epicanthal fold or how noticeable the eyelid crease is or isn't to begin with differs from person to person. However the principle can be reduced to addressing two key issues:<br />
-A decrease in the prominence of the epicanthal fold<br />
-Creation of a more noticeable eyelid crease<br />

In the right hands, this procedure yields fast and satisfying results to the patient. However it must be kept in mind that the immediate appearence postoperatively does not reflect the final outcome, which is true of so many other procedures in cosmetic plastic surgery.<br />
Other issues should be recognized, acknowledged and addressed prior to undergoing a blepharoplasty. Amongst these are the presence of asymmetry, eyelid ptosis(droopy eyelid), disfunction or dehiscence of the levator muscle of the eyelid causing droopiness for reasons other than mere excess tissue.<br />

The ideal eyebrow frames and enhances an expressive eye. A beautiful brow isn't simply a symmetric arch over the eye, it has a fluidity and symmetry to it. The head, body and tail compose a normal brow. It's shape is well depicted in <a href="http://nyccosmeticsurg.blogspot.com/2011/05/does-phi-make-beautiful-face.html"><span class="Apple-style-span" style="color: orange;">Marquardt's Golden Mask</span></a> as a line that progressively narrows laterally and peaks 2/3 of the way from mid to lateral points.<br />
In the world of make up, a narrow well groomed and darkened eyebrow is attractive. It's position is key.<br />
Here are the basic landmarks for the ideal female brow:<br />

</tbody></table>With age the brow begins to descend and droop over the brew bone and resting on the lid. The angle of the brow also changes, with the lateral higher portion slowly becoming horizontal in plane relative to the medial portion of the brow.<br />
How can plastic surgery correct this?<br />
There are several methods. Each one is tailored to specific needs. Some of the tools in the armamentarium of the plastic surgeon include:<br />
<span class="Apple-style-span" style="color: cyan;">-Botox to the lateral eye in the crows feet area. This helps reduce the crows feet and elevate the brow from persistent tonicity of the forehead muscles</span><br />
<span class="Apple-style-span" style="color: cyan;">-Endoscopic browlift: This is a minimally invasive technique through small holes behind the hair line through which a scope is inserted for visualization and dissection down to the orbit of the brow. This "free's up" the overlying tissues, and allows a lift with the use of absorbable anchors under the scalp.</span><br />
<span class="Apple-style-span" style="color: cyan;">-Excision (Surgical removal) of excess skin above the brow itself or at the hairline. This is the least preferred method since there can be residual visible scars.</span><br />

The female silhouette has been the inspiration for many works of art throughout the centuries. Amongst the many traits that make up a desirable and attractive physique are the buttocks. Sixteenth Century Artists such as Diego Velazquez portray's the female beauty and vanity eloquently in his painting titled "Venus at Her Mirror". Later, in the nineteenth century, French artists such as <span class="Apple-style-span" style="font-family: sans-serif; font-size: 13px; line-height: 19px;"> </span>William Bouguereau who in his realistic style depicted mythological themes flawlessly, also placed heavy emphasis on this part of the female human body. His ideal of perfection portrayed a slender juvenile and beautiful physique as seen in his work "Lost Pleiad" (1884)<br />
What is considered to be desirable in terms of shape and volume varies greatly from one individual to another and is undoubtedly, transcultural.<br />
The universal allure of the "hour glass figure" is more evident in certain cultures. In Latin and Afro american cultures, it is generally well accepted that an attractive back side is a very desirable trait. Among the many theories pertaining to the attractiveness of this physical trait in women, an evolutionary theory stands out. It is hardly sophisticated and assumes the hardwired sentiment equating it to reproductive potential and fertility.<br />
Popular culture, especially during the last decade has made no attempt to minimize it's fashionability. More recently, the media has increased the general demand for gluteal augmentation and reshaping. Statistics collected by the Society for Aesthetic Plastic Surgery reported a 533% increase in gluteal augmentation between 2002 and 2003.<br />

The further study of the buttock shape and anatomy in plastic surgery has designated this anatomical unit as the Gluteal Aesthetic Unit.(GAU)<br />
There are a variety of techniques in plastic surgery available to enhance the profile of the (GAU) but can be divided into 2 categories. Autologous (using the patients own tissue) augmentation and Prosthetic Augmentaion (silicone implants similar to breast implants)<br />
<br />
The brazilian butt lift is an Autologous tissue augmentation, because it uses excess fat from other portions of the body and then artfully injects them strategically into the areas above and directly into the gluteal muscle to enhance the buttocks. Total fat graft survival is reported to be 50% to 70%. MRI studies have showed favorable results after 1yr in terms of fat graft survival.<br />
<br />
<span class="Apple-style-span" style="color: orange; font-size: large;">Method</span><br />
The Plastic Surgery Literature is replete with a variety of methods for harvesting fat and then applying it to the gluteal area for augmentation.<br />
In general, the fat donor area is infiltrated (injected) with saline solution in combination with lidocaine and epinephrine(decreases bleeding). This combination is known as tumescent solution. Several versions of this solution are described in the plastic surgery literature. Once infiltrated, approximately 10-20 minutes are allowed to elapse before the fat is harvested into 60cc Syringes with a blunt tip cannula through a small incision. The typical donor sites are the abdomen and the hip areas or abdominal flanks (sides of the abdomen). Other potential donor sites are the upper thighs. Harvesting has also been described with standard liposuction techniques.The fat is allowed to sit and decant(float to the surface). Excess fluid is discarded. Once harvested, a small midline incision at the junction of both gluteus muscles is made. This site is not visible since it is in the crease. A cannula connected to a syringe containing the "purified" fat is inserted and injected strategically in a fan shaped pattern along the gluteal surface to selectively enhance it. Since 30-50% of the fat will resorb (not remain), this is added to the total expected amount of volume.<br />
The typical described amount of the injection varies greatly, but amounts ranging from 300 or 400 to 1500 have been described.<br />

Several studies have demonstrated favorable results using fat grafting. One study including 162 patients of which 123 were available for written followup had the following results:<br />

Breast augmentation has become the most popular cosmetic plastic surgery procedure over the past few years.<br />
By 2006 approximately 330,000 breast augmentations were performed in the US up approximately by 55% when compared to 2000.<br />
Silicone implants have been utilized since the 1960's and have been evolving ever since. 4 Years after the introduction of the silicone implant, the first inflatable saline implant appeared on the market and was later purchased by Mentor. Since then several generations of both silicone gel and saline implants have appeared on the market for widespread medical use.<br />
The safety of silicone gel implants became the subject of serious speculation during 1992 which lead to a moratorium on their use in the US. Since then, several large scale studies directed at clarifying the role of silicone implants in relation to connective tissue diseases have extended our understanding of these biomaterials and shown no causality between the two.<br />

After 1993 the fourth generation of silicone breast implants was introduced into the marketplace for general use. This generation differs from its predecesors in that it has a multilayered construction and much less "bleed" of silicone when ruptured. An increase in the crosslinkage of the silicone itself creating a "gummy bear" like material that retains it's shape better and is cohesive so as to not spill into adjacent tissues if ruptured it the current concept in comercial designs. Companies like Inamed, Allergan and Mentor, are at the continuosly propelling the development of more sophisticated designs that are aimed at a natural appearence and consistency.<br />
Fifth Generation Cohesive Gel implants are currently undergoing clinical trials.<br />

<span class="Apple-style-span" style="color: orange;">Do breast implants cause cancer?</span><br />
This is a common concern among women seeking Cosmetic procedures such as augmentation, augmentation/mastopexy and breast reconstruction after mastectomy. To date several large scale studies conducted internationally and in the United States have failed to find a connection between slilicone implants and Breast cancer.<br />
More recently, however attention was focused on the possible association of silicone breast implants and the development of a rare type of Lymphoma (lymphatic cancer of the breast) known as Anaplastic Large Cell Lymphoma (ALCL). 34 cases were identified world wide in a literature review quoted in the FDA's website, and a total of 60 cases have been reported in association to breast implants. There is ongoing research to elucidate if there is in fact a causal relationship and if so, what is the mechanism. It is important to keep in mind that this number represents a very minuscule percentage of the 5-10 million women who have breast implants worldwide.<br />

This decision is based on several factors and should address some important questions during the consult, such as..<br />
<span class="Apple-style-span" style="color: cyan;">-What is the ultimate goal for size and appearence that the patient desires from the procedure; Specific concerns and goals should be discussed with the patient about what the procedure will offer immediately postoperatively in addition, possible changes that may occur in the long run from months to years after the procedure.</span><br />
<span class="Apple-style-span" style="color: cyan;">-Is the patients skin quality such that it will accommodate a sub glandular implant versus a submuscular implant?</span><br />
<br />
Of the 4 approaches (Incision sites) available for augmentation, which would be the least visible and most desirable in the patients view. And which type of implant can be inserted through which approach?<br />
There are 4 accepted approaches for implant placement;<br />
1-The transumbilical approach (for saline implants only); this technique is less popular because it does not allow direct visualization of the plane of dissection and ultimately where the implant will lay. This limits the ability of the surgeon to precisely create a pocket that will accommodate the implant.<br />
2-Transaxillary approach (Both saline and small-medium silicone implants)<br />
3-Periareolar in which an incision is made at the edge of the areola (Both Saline and Silicone)<br />
4-Inframammary approach, which is located at the crease under the breast (Both Silicone and saline)<br />
<br />
Although silicone implants give increased patient satisfaction over all, if the cumulative evidence in favor of this type of material still leaves room for significant concern in the patients mind, a saline implant may be the best option.<br />
<br />
<span class="Apple-style-span" style="color: orange;">Implant rupture rates</span><br />
Prior generations of implants had a relatively high rate of rupture ranging from 0-69%. Newer generation implants have a much lower reported rate between 0.5% and 3.5%. One study quoted a 1% rupture rate in the Inamed 410 type implant. As new generations of implants emerge, the rupture rate will most likely improve, and leakage of cohesive material will facilitate implant exchange due to its mechanical advantage.<br />
Leakage of implants may be diagnosed clinically and also with MRI, CT, Mammography and Ultrasound,in different settings. MRI provides the highest sensitivity.<br />
If there is rupture of the implant, it is recommended to simply remove the implant and exchange it for a new one of the same size in the same surgery if there are no other synchronous morbidities at that time such as infection.<br />

Mammography remains the standard of care for monitoring the breast for potential breast cancer. A special technique called the Eklund technique consisting of displacement of the implant for appropriate visualization of the breast tissue is recommended. MRI is a valuable adjunct for monitoring the breast for cancer, and in addition is highly sensitive for diagnosing rupture of the implant when present.<br />

On profile, the upper lip should ideally project approximately 2 mm beyond the lower lip, which, in turn, projects 2 mm beyond the most anterior portion of the chin.<sup>1</sup><br />
The inclination of the lower lip is determined by it's fullness which also affects the appearence of the chin <br />

<span class="Apple-style-span">There are variety of methods for lip augmentation available in the armametarium of the cosmetic plastic surgeon. Although no fillers are currently approved by the FDA for lip augmentation, this is a common of label use for non-permanent fillers such as Hyaluronic Acid (HA).</span><br />
<span class="Apple-style-span">HA has the advantage of producing few side effects and being temporary, lasting up to a year.</span><br />
<span class="Apple-style-span">Permanent solutions with methylmethacrylate (PMMA) and ophtalmic grade silicone are also available, but are not as widely used. These permanent fillers may produce secondary complications such as granulomas which may require excision (removal) via surgery or liposuction.</span><br />
Fat grafting has gained popularity because it is autologous (from one's own body) and gives much longer lasting effects.<br />

During your initial consult you should ask everything you can about the procedure, make sure your plastic surgeon addresses any concern you may have regarding lip augmentation. Common questions are related to the shape and size of the augmentation, "is the procedure painful?" "How long will it take to recover and for the swelling to go down?"<br />
In addition to asking questions, most cosmetic plastic surgeons will evaluate the general health of the patient and take pre-procedure photographs and measurements of your lips. Always tell your Surgeon what medications you take, especially any that may be considered blood thinners, such as aspirin.<br />
Common medications that may cause bleeding during a procedure are:<br />
<br />
<ol><li>Aspirin or other antinflamatory medications such as Ibuprofen</li>
<li>Garlic supplements</li>
<li>Ginkgo biloba</li>
<li>Fish oils (Omega Fatty Acids)</li>
<li>Coumadin</li>
<li>Heparin or Lovenox</li>
<li>Vitamin E</li>
<li>Antibiotics</li>
</ol><br />
A mirror should be used for the patient to point out any areas of concern to be addressed on the lips. Make sure that your expectations of the procedure are congruent with what the procedure can provide. Always ask your surgeon what he/she recommends and what you should expect<br />
<br />
<span class="Apple-style-span" style="color: orange;">What to expect after augmentation</span><br />
Although in the scheme of things, lip augmentation is a smaller less invasive procedure than surgery, it will require some downtime afterward. This is something to consider in the busy hussle and bussle of NYC. I recommend 3-4 days to allow the local swelling to subside.<br />
There may also be temporary distortion of the lips. It is recommended to apply ice packs to the site to mitigate these side effects. Pain is well controlled by applying filler with local anesthesia and lasts 4-6 hours on average. Control of local discomfort for this procedure is usually addressed with NSAID type medications (eg. Ibuprofen) and patients tend to do well.<br />
<span style="color: orange;">Pitfalls of Augmentation</span><br />
Unfortunately, not all practitioners are familiar with what conforms with a natural youthfull appearence of attractive lips. Simply adding volume to the lips, often yields displeasing cosmetic results. Technical knowledge as well as a deep understanding of the regional anatomy is needed to acheive a good result. In addition to the previously mentioned, realistic expectations are equally important. Voluptuos lips are a very desirable trait, but the key to a succesfull outcome for maximum effect consists in applying "just enough" volume in just the right places.<br />
The safe approach to augmenting lips in terms of a natural appearance consists in being conservative. In this particular case, sometimes "less is more"<br />

<span class="Apple-style-span" style="color: orange; font-size: large;"> Hyaluronic Acid (HA)</span><br />
<span class="Apple-style-span" style="font-size: large;"> There are currently several FDA approved products for injectable dermal fillers. The most commonly used filler is Hyaluronic Acid (HA)</span><br />
<span class="Apple-style-span" style="font-size: large;"> HA is found naturally in human connective tissue as part of the living tissue matrix. It is synthesized and used for correction of moderate to severe facial wrinkles and folds.</span><br />
<span class="Apple-style-span" style="font-size: large;"> Examples of commercially available HA brands are Juvederm, Restalyne, Captique, Perlane, and Hylaform. </span><br />
<span class="Apple-style-span" style="font-size: large;">Early studies showed superior results of HA over collagen compounds. These presentations are non-animal stabilized HA and have less potential for having an allergic reaction, with the exception of Hylaform which does confer immunogenicity (potential for allergy).</span><br />
<span class="Apple-style-span" style="font-size: large;">Another advantage to the use of using (HA) is that since it binds to water, tissues undergo more distension, possibly leading to more native collagen formation 2 This, in theory translates into a longer lasting effect in terms of Wrinkle correction. </span><br />
<span class="Apple-style-span" style="font-size: large;"> Particle concentration and the duration of these substances in the injected tissues varies from one brand to the next.</span><br />
<br />
<span class="Apple-style-span" style="color: orange; font-size: large;"> Calcium hydroxylapatite (CaHA)</span><br />
<span class="Apple-style-span" style="font-size: large;">This is the mineral component normally found in teeth and bone.</span><br />
<span class="Apple-style-span" style="font-size: large;">Radiesse is a suspension of 30% CaHA microspheres.</span><br />
<span class="Apple-style-span" style="font-size: large;">This substance has been found to persist in tissues for 2 to 7 years. It was FDA approved in 2006 for a variety of conditions including soft tissue wrinkles and folds, but allows for versatility in its use. </span><br />
<span class="Apple-style-span" style="font-size: large;">Other uses include correction of lipoatrophy seen in HIV patients, correction of dental interosseus defects and laryngeal augmentation.</span><br />
<span class="Apple-style-span" style="font-size: large;"> CaHA is degraded over time, and has not been shown to create a granulomatous foreign body reaction 3,4,5</span><br />
<span class="Apple-style-span" style="font-size: large;"> The clinical effect lasts approximately 12 months </span><br />

<span class="Apple-style-span" style="color: orange; font-size: large;"> Poly-L Lactic Acid</span><br />
<span class="Apple-style-span" style="font-size: large;">Poly - Lactic PLA (Sculptra)Acid is a synthetic material first made in 1954 . It has a wide variety of applications in the field of surgery. When used as a dermal filler, it persists in the tissues for 9 months. However it is substituted by collagen ingrowth (neocollagenosis) thus allowing for persistent filling of the Rhytid (Wrinkle). It has been used with success in treating HIV related lipoatrophy of the cheeks(Sunken cheek appearence). It has been shown to have a three fold increase in dermal thickness in this population, improving the longevity of the beneficial effects. The clinical benefits last approximately 2 years.</span><br />

<span class="Apple-style-span" style="color: orange; font-size: large;"> Polymethylmethacrylate beads coated in collagen</span><br />
<br />
<br />
<span class="Apple-style-span" style="font-size: large;"> Polymethylmethacrylate (PMMA) is a widely used material chemically similar to plexy glass. It is a non-degradable and permanent filler. The commercial name for this filler is Artefill, and it is composed of 20% PMMA with bovine collagen . The bovine collagen surrounds the PMMA microspheres and is ultimately substituted by native collagen. Artefill was FDA approved in 2007 for the treatment of Rhytides. Since it has bovine collagen, it may result in an allergic reaction. Skin sensitivity testing is usually done prior to its application. When applied skillfully, this filler has a very pleasing appearance clinically. Since PMMA cannot be metabolized or Phagocytized (Cellular removal of foreign body) the beneficial effects are permanent and often only show as the patient ages.</span><br />
<br />
<span class="Apple-style-span" style="font-size: large;"> One of the potential disadvantages of this filler is that once it is injected, the result is final even if the final appearance is undesirable. In addition to that, it may induce a local reaction and form indurated areas known as granulomas. </span><br />
<br />
<span class="Apple-style-span" style="font-size: large;"> Granulomas often require surgical excision, either directly of via liposuction.</span><br />
